IF YOUR ANSWER TO ANY OF THESE QUESTIONS IS YES, DO NOT COME TO THE CAMP.

The reason behind this set of questions is being able to pre-screen our players and their families. If we can eliminate those who are openly symptomatic or may have been exposed, then we dramatically lessen the risk of infection at our facility. This will lead to a better sense of safety for all. We hope to cut down on those who could carry the virus from entering the facility, but it is not absolute. It is a further layer of protection.

NOTE: The confidentiality of all medical information will be respected, and information will not be disclosed except as otherwise required and provided here-under to effectuate the purposes and provisions of this health and safety plan.
